day is for you, how long you have had RSD, the various treatments you have had, etc.

you wish to, such as sex, and age, and state you live in.
3) Explain why you think it is important that Any of these

below should have a show about RSD. How it would help yourself, other RSD patients, and the RSD Community in general.
4) If you would like, include a website that they can go to for more information.
